“The Learning Center will serve as the Program Office for the DHCS’ Equity and Practice Transformation (EPT) program. EPT is a five year, $700 million primary care practice transformation program. It aims to advance health equity and address COVID-19-driven care disparities, enhancing the health and wellness of Medi-Cal enrollees. “
Population Health Learning Center was a brand new California wide population health nonprofit organization that was in need of a quick branding and website launch. We worked on getting some brand colors, fonts and then a brand mark and logo. Then we moved on to finding photography that represented their brand and then to the building of a simple, but informative, website.
